Location Notes: Pre-visit health screenings must be performed for the client. Before entering the room for the start of your shift, please confirm the client is symptom free by standing 6 feet away and asking the caregiver on duty the health screening questions.

Trash is taken out once a shift. If client has incontinence issues the caregiver may tie it up, set it outside the door and call the front office. They will come by and pick it up. Med techs are the only ones allowed to go into the rooms while the residents are on observation. Only one person per shift is designated to the clients on observation, that person is our caregiver. For any immediate response for dishes or trash, call the front desk.

Client is located in Life Guidance (Memory Care) hall. Please enter through the front door where you will be signed in and screened. If the door is locked, call the facility at 281-359-8959 and let them know you are a caregiver there for your client. If no one answers, walk to the right side of the building and knock on the door that is in the middle of the building. They will notify the front desk that you would like to enter.

Summary: This client is a new resident of Atria Senior Living in Kingwood and must be monitored for 14 days before joining their community. During the monitoring period, the client and our caregivers will be confined to the clients room. Interaction from Atria staff will be limited.

  • Please complete the observation form before the end of each shift.

  • You will be screened and your temperature taken before and possibly during your shift.

  • You MUST wear a mask at all times.

  • You or the client are not allowed to leave the room.

  • Understand that there is risk to the Atria residents from our caregivers entering the building. Practice infection control, wash your hands, social distance and notify your supervisor if you experience, or have been around someone, who is experiencing upper respiratory symptoms like cough, shortness of breath or fever.

Stated Medical Condition - Dementia. Some of our caregivers have made note Charles has made advances toward them. Please remember he is a memory care patient and treat him as such. If you are uncomfortable, notify supervisor.

Notes

  • 5/8/2020 - It was reported the client made sexual advances to a caregiver. Please redirect patient and report future incidents to your supervisor through instant family updates.

  • 5/9/2020 - It was reported by the day caregiver that Charles had a cough and sore throat.

    • The nurse on duty took his temperature and it was normal. They will continue to monitor him and was prescribed an allergy medicine.

    • As a precaution, the facility has provided full PPE for our caregivers to use when performing personal care.

      • 1 - n95 mask per shift

      • 2 - Gown per shift

      • 1 - Set of goggles if client coughing. Please clean with soap and water after use.

      • Disinfectant wipes, hand sanitizer and gloves are supplied by the facility. Submit an instant family update when supplies begin to run low.

      • The facility will allow the caregiver to sit outside of the room if they are not comfortable staying in the room until his Covid test comes back negative.

    • You must use universal precautions and PPE when performing personal care and transferring.

    • The client must wear a provided surgical mask if he is coughing.

  • 5/10/2020 - Reports from the facility director is the medication has helped and is symptom free.

  • 5/14/2020 - Pre-visit health screenings must be performed for the client. Before entering the room for the start of your shift, please confirm the client is symptom free by asking the caregiver on duty.
